Insurance Billing & Payment Information
It is important that our patients understand that they will typically receive three separate bills if they are utilizing a payment method other than our Bundled Self Pay Plan. The three bills required to be paid are the facility fees, anesthesia fees, and surgeon fees. The surgeon, anesthesiologist, and certified nurse anesthetist bill for their expertise and time. This is the common method of billing regardless of the location in which you receive treatment.
The facility is responsible for covering all the costs of supplies, overhead, specialized tools, and staffing. For this reason the facility portion will be your highest bill. Depending on your specific surgical needs, you may also receive a bill for any laboratory, pathology, or diagnostic services that you receive.
Depending on your coverage, you may be asked for payment prior to your surgery. We make every effort to inform you of this amount prior to your admission. When you arrive for surgery, you should be prepared to pay all co-payments and your deductible if it has not been met.
For your convenience, we accept MasterCard, VISA, American Express, and Discover credit cards. We also accept cashier’s checks, personal checks, and money orders.
If you require assistance with making a payment, we are happy to work out a payment plan. If your out-of-pocket bill is too high to justify our risk of setting up a payment plan, then you are more than welcome to click here to learn more about ePay Healthcare’s surgical funding program. This loan can be used for a portion or for your entire surgical bill, including your surgeon and anesthesia costs.
If we need additional insurance information, or if you need to make a payment at the time of your surgery, one of our business office team members will contact you prior to your procedure. He or she will collect necessary insurance information to assist with your registration process.

Patients and prospective patients may request from this facility a formal hardcopy estimate of your surgical costs. We are a separate entity from your physician’s office and the anesthesiologists. Patients and prospective patients should contact each health care practitioner who will provide services in the ASC to determine network participation and estimate out of pocket costs.
